Building Public Outreach Programs

MEETING FORMS

The forms below must be completed and submitted no later than two months prior  to the MA annual meeting.  

MA Annual Meeting Coordination Form 
Complete/submit the form to your dedicated WEF Staff Liaison. 

WEF Materials Form 
Complete/submit the order form to Dianne Crilley by six (6) weeks prior to the MA annual meeting in order to guarantee "on time" arrival.

Awards Nomination Form and Information
Nominations for Bedell, Hatfield,  Burke and Laboratory Analyst Awards  must be submitted to Kelsey Hurst no later than eight (8) weeks prior to the meeting in order to guarantee on time arrival.  NOTE:  WEF Delegate service awards and Quarter Century Operator Club plaques and pins are ordered automatically by WEF and shipped to the MA for presentation at the MA annual meeting.    


MA ANNUAL REPORT

As soon as your meeting is completed, we need to receive updated information on your Board and Committee Chairs, to ensure that our communications reach the appropriate MA leaders.  

Within two (2) weeks following your MA Annual Meeting, please fill out the 2017 MA Annual Report , and submit to Dianne Crilley.  The form is interactive.  Fill out, save, and email to WEF.  Please include names of Board and Committee Chairs either in a link to your website or through a Word or excel document.  Also Include current information on Student Chapter Faculty advisors.
